Due to space limitation, registration will proceed on a first-come and first-served basis until space is filled. Please register early.

FEES

Early Rate (payment received on or before August 15, 2007):

Companies: $400

Academia/Research Institutes: $250

Students (pre-doctoral): $150

Regular Rate (payment received after August 15, 2007):

Companies: $450

Academia/Research Institutes: $280

Students (pre-doctoral): $180

Registration fees cover the following:

Wednesday, Oct. 10 Reception

Thursday, Oct. 11 Dinner

Breakfast, refreshment breaks and lunch on Thursday and Friday

PAYMENT POLICY

Payment must be made in US funds and received in order to process your registration. Payment by credit card is strongly preferred (Visa, MasterCard, AmEx). If paying by check, we will require a credit card to reserve your place at the program. Please make check payable to Harvard School of Public Health-CCPE. Space is limited. Note: Please do not make non-refundable flight arrangements until you have received confirmation from us.

CANCELLATION POLICY

All requests for cancellations must be made in writing. Cancellation on or before August 31, 2007 will be issued a refund less $40 administrative fee per person. Cancellations received between September 1, 2007 and September 14, 2007 will be issued a refund of 50%. After September 14, 2007, no refund will be issued. There will be no exceptions to this policy.
